Use our "Virtual Information Desk", accessible at the top of any page on our website by clicking on this Visitor Information Panel icon icon, to help answer common questions or help you find your way before and during your hospital visit.

Strengthening Primary Care Connections for Patients

We’ve made a commitment to play a leading role in collaborating with our community partners, patients and their families in creating a coordinated network of services within our Community – both inside the Health Centre and in partnership with other health care providers and social service agencies in West Toronto. Creating seamless transitions for patients is a strategic priority for St. Joe’s, and one that will help make our community one of the healthiest in Canada.

At the core of this work, is creating partnerships and better connections with primary care and community care providers so patients with complex health care needs are fully supported. It’s about helping to coordinate care that wraps around the patient in our community to help them get the help they need inside and outside of the hospital.

Working with the Toronto Central LHIN’s Integrated Primary and Community Care strategy and the Ministry of Health’s Patients First Action Plan for Health Care mandate, is a natural fit to our promise to patients and families to create a more coordinated and safer health care journey that for most people, begins and ends in the community.

Providing quality and safe care to our patients also means working closely with their care providers in the community before our patients come through our doors, a role that is important to our Vision of Advancing the Health of Our Community by Being Canada’s Best Community Teaching Health Centre.

That is why St. Joe’s is pleased to be the Hospital Resource Partner for the Toronto Central LHIN’s West Toronto Sub-Region, working alongside primary care and community care providers to coordinate initiatives that will improve care and preventative care  for patients.

Strengthening the connections between primary care providers and the health care system is the next step to ensuring better care for Ontarians and their families, and locally in West Toronto to the people who rely on us for care when they need it the most as their community hospital.
