St. Joseph's Health Centre, Unity Health Toronto is excited to announce the opening a 30 bed Transitional Care and Reactivation Care Unit located at Humber Church site (200 Church St.; near Jane Street south of the 401). The Reactivation Care Unit will provide care for adult and geriatric patients who require therapy, reactivation and transition planning in order to transfer to the community and/or other supportive environments. We are now recruiting a dynamic interprofessional team to deliver care to, and with, the patients in this program!

Position Summary:

The Nurse Practitioner (NP), Reactivation Unit, works with the inter-professional team and physician partners in the Medicine & Senior's Care Program to meet the unique needs of our patients. With a focus on medical management and optimizing function and discharge, the NP works to full scope of practice within the Reactivation Unit to support the medical and psychosocial needs of the patients.

The NP provides leadership and promotes excellence in nursing through the roles of practitioner, consultant, educator, researcher, and leader. By utilizing theoretical and clinical expertise in this specialty area, the NP delivers advanced practice nursing to patients and families with complex situations within the in-patient setting.

The NP contributes to the education and professional development of nursing/inter-professional staff, students and the professional community within these areas of specialization, and participates in academic and research projects. The NP integrates research into practice to ensure best practice standards are maintained and incorporated into clinical initiatives.

Responsibilities:
The Nurse Practitioner (NP) will utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for inpatients on the Reactivation Unit within a philosophy of patient-centered care. This will involve health assessment and diagnosis of medical and cognitive problems, developing and implementing a plan of care, providing health counseling and emotional support in the areas of health promotion and the prevention of illness, injury and complications.

Key areas of responsibility include:

Clinical Practice:
• Uses theoretical and evidence-based practice to plan, implement and evaluate advanced nursing care for patients within the Geriatric program
• Obtains and documents a comprehensive health history, performs a physical examination on patients requiring acute geriatric assessment, collaborates closely with the Geriatricians and other members of the Interdisciplinary team, and coordinates care involving other healthcare providers.
• Prescribes and monitors drug therapy and orders and interprets appropriate diagnostic and laboratory tests within the current legislative framework and internally developed and approved authorizing mechanisms such as medical directives
• Leads communication between patients and health care team when clinical, ethical or moral issues arise
• Partners with patients and families to ensure their priorities and needs are being addressed by the health care team

Consultation/Collaboration:
• Provides consultation internally for clinical practice issues within the NP's area of specialty and scope of practice. Consults with physicians, and refers to specialists and other team members when appropriate
• Collaborates and participates in planning, promoting, implementing and evaluating new ideas, concepts, and interventions to facilitate care to patients and families

Leadership:
• Participates in professional, program and hospital-wide Quality Improvement initiatives
• Actively promotes professional role through participation in external forums such as the Nurse Practitioner Association of Ontario
• Examines current processes, identifies opportunities for change and provides recommendations for quality improvement as necessary
• Provides leadership in the development/revision of practice guidelines, protocols, policies and procedures

Education & Scholarship:
• Participates in teaching of NP and MN nursing students
• Participates in clinical education opportunities that contribute to nursing practice excellence and interprofessional collaboration and education
• Participates in research through advancing research in relevant areas of practice, appraising research and disseminating research and innovative, evidence based practices

Qualifications:
1. Current registration in the Extended Class (Adult or Primary Care NP) with the College of Nurses of Ontario required.
2. Current Certificate of Competence with the College of Nurses of Ontario required.
3. Master's Degree in Nursing required.
4. Current Basic Life Support (BLS) required.
5. Successful completion of NP Prescribing of Controlled Substances exam (preferred).
6. Minimum of 3 years recent experience in adult nursing.
7. Excellent advanced clinical skills in the area of geriatric and general medicine nursing with the ability to provide care to a broad spectrum of patients in an interprofessinal care setting.
8. Membership in related professional associations required, e.g. (NPAO, RNAO, Geriatric Nursing Association membership, etc) is preferred.
9. Demonstrated evidence of sound organizational and prioritization skills with the ability to work well in stressful situations.
10. Proven ability to work in a collaborative setting, handle conflict situations and employ skills in diplomacy while networking in a collegial and productive manner.
11. Excellent communication, negotiation, teaching and interpersonal skills with the ability to work well with patients, families, all levels of staff and the community.
12. Ability to work well autonomously.
13. Demonstrated excellent oral and written communication skills, knowledge of the principles of adult education.
14. Satisfactory attendance record.
