$23.040 - $24.187 per hour

Family Medicine Centre (30 the Queensway site)/ Urban Family Health Team (27 Roncesvalles Avenue site)

Position Profile
The Family Medicine Centre/Urban Family Health Team assistant supports the family physicians, nurse practitioner and all our interprofessional team members in providing comprehensive and safe primary care services to our patients and their families. She/he will be responsible for preparing patient exam rooms, portering lab results, instruments and patients, ordering medical supplies and patient education materials as well as providing administrative support as required.

Qualifications include:
1. Ontario Secondary School Graduation Diploma or equivalent.
2. Current Personal Support Worker Certificate from a Community College or an Accredited PSW Education Program*.
3. Current certification in Basic Life Support, Basic Rescue Level C, or Heartsaver Plus issued by SJHC.
4. Computer skills including: email and internet, data entry, and system navigation (EMR) required.
5. Demonstrates excellent interpersonal and communication skills, and communicates fluently and
6. Effectively in the English language, both verbal and written skills.
7. Ability to set priorities and work under close or minimum supervision.
8. Ability to follow specific directions - For example; tray cleaning and wrapping and preparation.
9. Able to carry out detailed processes in a complex, fast-paced work environment.
10. Ability to perform FULL range of duties in this position including specific types of patient care as directed within the list approved for the category and by the Regulated Health Professions Act(RHPA).
11. Ability to provide patient observation, portering and transportation services for patients and supplies.
12. Knowledge of basic supply processing and distribution functions preferred.
13. Satisfactory attendance record.

*Candidates who are currently working at the Health Centre in the Health Care Attendant Job
Classification or in an equivalent Classification will be considered.
