It’s been two years since we launched our 2015-2020 Strategic Plan that outlines our vision to Advance the Health of Our Community by Being Canada’s Best Community Teaching Health Centre. Since then, our teams have made significant progress, implementing initiatives that are enhancing the care we provide to our patients and the community.
Here are some of our recent accomplishments:
CREATING SEAMLESS TRANSITIONS OF CARE
We committed to work with community and home-based healthcare providers to make sure patients have the right care at the right time.
DELIVERING AN EXCEPTIONAL PATIENT EXPERIENCE
Patients are happier and healthier when they are involved in their care. We’ve challenged ourselves to integrate the patient perspective into everything that we do.

SHAPING CARE THROUGH TEACHING AND EDUCATION
We renewed our commitment to create a culture of learning by investing in professional development for our people and developing new tools to help patients to better manage and recover from illness.
