Celebrating Our Successes

It’s been two years since we launched our 2015-2020 Strategic Plan that outlines our vision to Advance the Health of Our Community by Being Canada’s Best Community Teaching Health Centre. Since then, our teams have made significant progress, implementing initiatives that are enhancing the care we provide to our patients and the community.

Here are some of our recent accomplishments:


We committed to work with community and home-based healthcare providers to make sure patients have the right care at the right time.

wheelsPiloted a new approach to care for patients with complex needs – developed 445 coordinated care plans to better support them

pplhandPartnered with 13 healthcare organizations to connect vulnerable patients to social services

schoolHad 140 visits to our new paediatric school clinic in the seven months since it opened

ppl323 patients have been better prepared to go home by using our new discharge process
