A new resource is helping patients receive faster care by connecting their family physicians with specialists at St. Joe’s.
SCOPE (Seamless Care Optimizing the Patient Experience) is a Toronto-born initiative that allows community physicians to dial one number and have access to community resources and health-care professionals, including internal medicine specialists and our diagnostic imaging team.
“I’ve found it to be amazing,” said Dr. Jenny Clement, a family doctor who works in our community. “I used it today with great success – it helped me keep somebody out of the Emergency Department.”
Typically when a patient has a specific need such as an urgent diagnostic test, their family doctor may feel that they have no option but to recommend that they go to the Emergency Department (ED) to obtain this. SCOPE allows them to access such urgent tests in a more appropriate way, giving doctors the ability to speak with Diagnostic Imaging directly.
“My patient needed an urgent cystoscopy and would have otherwise had to go back to the ED in order to have that set up,” Dr. Clement said. “He’s got mobility issues so it would have been very challenging for his caregivers. Thanks to SCOPE, I was able to book him a test for next week.”
“As a community Health Centre, we have a close relationship with family doctors in our community,” said Dr. Dan Cass, Vice President, Medical and Chief Medical Officer at St. Joe’s. “Working together allows us to make sure patients are receiving the care they need where they need it, and that they’re transitioning between health care providers in the most convenient way possible.”
In addition to diagnostic imaging, SCOPE also provides access to internal medicine specialists and medicine subspecialists, a nurse navigator and CCAC (Community Care Access Centre). Sponsored by the Toronto Central LHIN, SCOPE is currently being used by 20 family doctors in our community, with the goal of rolling it out to more family doctors in our neighbourhood later this year.
