The Resource Manual is designed for use by healthcare providers interested in being trained as coaches for healthcare teams to advance collaboration among healthcare professionals. The goals is to support team coaches in achieving success in facilitating and sustaining interprofessional collaboration in their organizations. The Resource Manual is a learning tool for team coaches in healthcare settings, and is intended to complement the Team Coach Training Program (which is offered by St. Joseph’s Health Centre).

A Roadmap for Co-Creating Interprofessional Models of Care

Interprofessional practice occurs when different professions come together for the purpose of patient-centred healthcare delivery. The purpose of this Toolkit is to provide leaders and clinicians working in a variety of healthcare settings with a set of guidelines and tools that will enable the enhancement and/or development of interprofessional teams.

The Toolkit provides a framework which organizations and individuals can use to engage with one another in the co-creation of interprofessional models of care and can be used to help move interprofessional practice from conceptualization to actualization.
